Contraindications

Lidocaine hydrochloride: Contraindicated for those allergic to local anesthetics. Intravenous contraindication in patients with Adams' syndrome (acute cardiogenic cerebral ischemia syndrome), pre-excitation syndrome, and severe heart block (including sinoatrial, atrioventricular and intraventricular block).

Lidocaine anesthetic powder precautions

1. To prevent accidental entry into blood vessels, pay attention to the symptoms of local anesthetic poisoning.

2. During the medication, attention should be paid to check blood pressure, monitor the electrocardiogram, and be equipped with rescue equipment; if the P-R interval of the electrocardiogram is prolonged or the QRS wave is widened, and other arrhythmias occur or the original arrhythmia is aggravated, the drug should be discontinued immediately.

3. Use with caution in the following situations: 

(1) Congestive heart failure.

(2) Severe myocardial damage.

(3) Severe sinus bradycardia. 

(4) Incomplete atrioventricular block or intraventricular block.

 

Lidocaine anesthetic powder Dosage

(1) Dosage for adults

 

①Topical anesthesia: 2% to 4% solution should not exceed 100mg at a time. When administered by injection, the dose should not exceed 4.5 mg/kg (without epinephrine) or 7 mg/kg (with 1:200,000 concentration of epinephrine).

 

② caudal block for labor analgesia: use 1.0% solution, limited to 200 mg.

 

③ Epidural block: use 1.5-2.0% solution for thoracolumbar segment, 250-300 mg.

 

④ Infiltration anesthesia or intravenous injection of regional block: use 0.25 ~ 0.5% solution, 50 ~ 300mg.

 

⑤Peripheral nerve block: 1.5% solution for brachial plexus (unilateral), 250-300 mg; 2% solution for dental use, 20-100 mg; 1% solution for intercostal nerve (each branch), 30 mg, limited to 300 mg; cervical For parainfiltration, use 0.5%-1.0% solution, 100 mg on the left and right sides; use 1.0% solution for paravertebral spinal nerve block (each branch), 30-50 mg, limited to 300 mg; use 0.5-1.0% solution for pudendal nerve, 100 mg on the left and right sides .

 

⑥ Sympathetic ganglion block: cervical stellate nerve with 1.0% solution, 50mg; spinal anesthesia with 1.0% solution, 50 ~ 100mg.

 

⑦ One-time limit, 200 mg (4 mg/kg) without adrenal, and 300-350 mg (6 mg/kg) with epinephrine; regional block of intravenous injection, maximum dose of 4 mg/kg; intravenous injection for treatment, the first initial dose 1～2mg/kg, the maximum dose is 4mg/kg, and the adult intravenous infusion is limited to 1mg per minute; repeated multiple doses, the interval should not be shorter than 45～60 minutes.

 

(2) Common dosage for children

Depending on the individual, the total amount of one dose should not exceed 4.0-4.5 mg/kg, 0.25-0.5% solution is commonly used, and 1% solution is used in special cases.
