Description
Product application of Selpercatinib:
1. Treatment of RET fusion-positive non-small cell lung cancer (NSCLC): Selpercatinib is a highly selective and potent inhibitor of RET kinase activity. RET fusion oncogenes are identified in approximately 1-2% of NSCLC patients. Selpercatinib has shown significant efficacy in clinical trials as a first-line and second-line therapy in RET fusion-positive NSCLC patients, leading to improved overall response rate (ORR), progression-free survival (PFS) and overall survival (OS).

2. Treatment of RET-mutant medullary thyroid cancer (MTC): MTC is a rare subtype of thyroid cancer, which is characterized by the activation of oncogenic RET mutations. Selpercatinib is approved for the treatment of patients with metastatic MTC, who have positive RET mutations. It has demonstrated improved responses in clinical trials, leading to ORR of 69% and duration of response (DOR) of 22.8 months in the Phase I/II ARROW trial.
3. Treatment of RET fusion-positive thyroid cancers: RET fusions are also identified in a subset of thyroid cancers, such as papillary thyroid cancer (PTC) and poorly differentiated thyroid cancer (PDTC). Selpercatinib has shown promising responses in RET fusion-positive thyroid cancers, leading to ORR of up to 73% in Phase I/II trials. It has the potential to be used as a targeted therapy for these types of cancers in the future.
4. Management of Adverse Events (AEs): Selpercatinib is generally well-tolerated, with a manageable safety profile. However, like any other drug, it may cause AEs, including hypertension, diarrhoea, fatigue, abdominal pain, and increased levels of liver enzymes, among others. Monitoring for AEs and adjusting dosing accordingly are critical to mitigate their impact on patients' quality of life.
5. Identification of RET alterations: Selpercatinib is a companion diagnostic agent designed to identify the presence of RET alterations in tumour tissue or blood samples. This tool allows physicians to select the appropriate patients who may benefit from selpercatinib therapy.
6. Comparison with other TRK inhibitors: Selpercatinib is a different class of drug from TRK inhibitors, which target tropomyosin receptor kinase (TRK). RET and TRK are distinct receptor tyrosine kinase (RTK) families, with different structure and functions. Unlike TRK inhibitors, selpercatinib selectively inhibits RET activity and has no significant activity against other RTKs. Therefore, selpercatinib has a more specific targeting of RET oncogenes without affecting other signalling pathways, which may result in improved efficacy and safety profiles.
7. Combination therapy: Selpercatinib can be used as monotherapy or in combination with other agents. Preclinical studies suggest that the combination of selpercatinib and other targeted therapies, such as immune checkpoint inhibitors, may enhance antitumour activity and overcome resistance mechanisms. Clinical trials are ongoing to investigate the safety and efficacy of selpercatinib combination therapy.
8. Potential for use in other solid tumours: RET alterations are also found in other solid tumours, such as colorectal cancer, breast cancer, and pancreatic cancer. Preclinical studies suggest that selpercatinib may have antitumour activity in these tumour types. Clinical trials are ongoing to further investigate the potential of selpercatinib in these cancers.
9. Cost and availability: Selpercatinib is an expensive drug, with a list price of approximately $20,000 per month of therapy. However, it is covered by many insurance plans, and financial assistance may be available for eligible patients. Selpercatinib is currently available in limited markets worldwide. However, as more clinical data are gathered and regulatory approvals are obtained, it is expected to be available in more countries soon.
10. Future directions: The development of targeted therapies, such as selpercatinib, has revolutionized the treatment of cancer. The identification of specific oncogenic alterations, and the development of drugs that selectively inhibit their activity, has transformed the way oncologists approach cancer treatment. However, more research is needed to optimize the use of these therapies, improve their safety and efficacy, and identify additional therapeutic targets. The future of oncology lies in precision medicine, a paradigm where each patient's cancer is specifically molecularly profiled, and an optimal therapy is selected based on individual genetic makeup.
