Apixaban is an orally active direct anticoagulant and a selective factor Xa inhibitor. It belongs to the class of direct oral anticoagulants (DOACs) and is used in the prevention and treatment of blood clot-related conditions. Mechanism of Action
The coagulation cascade involves a series of enzymatic reactions that ultimately generate thrombin. Factor Xa plays an important role in this process by converting prothrombin (factor II) into thrombin (factor IIa). Thrombin then promotes the conversion of fibrinogen into fibrin, which contributes to the formation and stabilization of blood clots.
Apixaban works by selectively and directly inhibiting factor Xa. It can inhibit both free factor Xa and factor Xa associated with blood clots. By reducing factor Xa activity, apixaban decreases thrombin generation and subsequently reduces fibrin formation. This anticoagulant action helps limit the formation and progression of blood clots.
Apixaban does not directly inhibit platelet aggregation. Its anticoagulant effect mainly results from interference with the coagulation cascade rather than direct effects on platelets.
Function and uses
Anticoagulant Function: The primary function of apixaban is anticoagulation. By inhibiting factor Xa, it reduces thrombin generation and may help prevent excessive clot formation.
Stroke and Systemic Embolism Prevention: In adults with nonvalvular atrial fibrillation, abnormal heart rhythm can increase the likelihood of blood clot formation and subsequent embolic events. Apixaban is used to reduce the risk of stroke and systemic embolism in this setting.
Venous Thromboembolism Management: Apixaban is also used in the treatment of deep vein thrombosis (DVT) and pulmonary embolism (PE). It can also be used to reduce the risk of recurrent DVT and PE following initial treatment.
Post-Surgical Thrombosis Prevention: Apixaban has also been approved for reducing the risk of DVT following certain hip and knee replacement procedures.
